Trustee / Beneficiary/LLC Form

If your Eastham property is held in a Trust or an LLC and you would like to get beach and transfer station stickers for the trustees/beneficiaries of said trust/LLC members, please upload the relevant trust pages/LLC operating agreements. 

NOTE: Successor trustees are not beneficiaries. A schedule of beneficiaries is usually required, or a distribution of shares along with the page that lists the beneficiaries by name. As there are a variety of ways to set up a trust depending on the purpose of each trust, it is the responsibility of the taxpayer to find the required pages.

If you do not have an operating agreement, please fill out and upload an Affidavit of LLC membership.

  14. Please list all the LLC members that will apply for beach stickers. 

  16. If the property is held by a trust, please upload relevant pages of your trust that list current trustees and the beneficiaries. 

    If the property is held by an LLC, please upload the operating agreement for the LLC. If you do not have an operating agreement, please fill out the Affidavit of LLC Membership and upload it here.
